Abstract

Uncontrolled hypertension remains the leading modifiable driver of cardiovascular mortality in West African cities, yet primary care systems rarely retain patients beyond the first two visits. We tested whether structured monthly follow-up by trained community health workers improves blood pressure control at twelve months compared with standard clinic-based care. Twenty-four primary health centres in peri-urban Lagos were randomised, enrolling 1,842 adults with a confirmed diagnosis. At twelve months, 61.4% of intervention participants had achieved control against 38.9% in the control arm. Retention in care was 84% versus 52%. Task-shifted follow-up is a low-cost, deliverable addition to existing primary care in dense urban settlements.
